This patient had silicone injected into the lips, which is another common area for injection.  Patient perceived the area as hard and abnormal.  Dr. Hughes made a hidden incision on the inside of the lip and the was able to remove the tiny amount of residual silicone as well as scar tissue that was resulting in this perception of hardness.
